Ms. Betty Bittinger is a sixty-three year old widowed woman who has been treated at the stevens center for several years for chronic undifferentiated schizophrenia. she has been maintained on a long-acting form of Haldol, which she receives by injection on a monthly basis. she has had several psychiatric hospitalizations over the years for her schizophrenia, and has reconstituted fairly well between hospitalizations. Following her husband's death in December of 1995, Ms. Bittinger developed a complicated grief reaction, basically meaning that rather than experiencing a "normal" grief reaction after a loss, she developed a significant depression. During this time, Ms. Bittinger's function deteriorated significantly. Her husband apparently was a major source of support for Ms. Bittinger up to that point in time. Her depression deteriorated to a point where she required inpatient hospitalization, so that the medications which were being used to treat her conditions could be adjusted under closer supervision than outpatient treatment would allow. she was discharged on 100 mgs of pamelor, an antidepressant, along with Haldol 5 mgs orally twice per day. she continues to take these medications and to receive Haldol Decanoate 100 mgs once per month. MS. Bittinger was last seen by myself on June 3rd, and was no longer depressed. She no longer experiences the psychotic symptoms which accompanied her depression (primarily in the form of thinking she heard her deceased husband's voice, and seeing visions of him). she was well-dressed and groomed. cooperative, and with a cheerful affect. A brief cognitive exam was performed which revealed her to be alert and oriented in all spheres. Her short, immediate, and long-term memory were intact. Her concentration was fair. she noted that she felt she was getting along well at the residence where she currently is living, and in that supervised setting is taking care of her hygiene and activities of daily living. Ms. Bittinger has never managed her finances alone, to my knowledge, and seems to accept that she will always require some assistance in this area.
