The petition of Jose Simental respectfully statesl
1. Mr." Mrs. Oscar Caban are the aunt and uncle of
Pe ti tioner.
2. Jose Simental was born on April 16, 1981 and resides at
12ll Petersburg Hoad, Carlisle, Cumberland County with his aunt
and uncle.
3. Jose Simental is the son of Lois Castillo who will be
moving to Mexico for approximately one year.
Mrs. Castillo is
Mr. Caban's sister and has authorized Mr. Caban to act as
guardian of Petitioner. (See Exhibit Band C attached).
4.
Mr.
Caban is a disability retiree of the U.S. Army
and is authorized medical care by the Army physicians fO;7 himself
and members of his household. The Army will not grant medical
serv ices to Mr. Caban's nephew unless Mr. Caban is appointed
guardian by a Court of competent jurisdiction.
5. No other guardian has been appointed of the person of
the petitioner.
6. The proposed guardian is Oscar Caban, uncle of the
minor, who is 42 years of age and resides at 120 Petersburg Road,
Carlisle, Cumberland County.
His acceptance is attached as
Exhibit "A".
7 . The proposed guardian has no interest adverse to the
minor.
