As a result of his father's death, minor is the beneficiary of approximately $ in life insurance proceeds from Ollice of Servieemen's Group Life Insurance. The company will not disburse funds except to a court-appointed guardian of the minor. 9. At the present time minor has no eourt-appointed guardian. 10. Petitioners seek appointment as guardian of the minor's estate. 11. Both parties shall utilize child restraint seats nnd seat belts when tranSPorting the child by automobile. S. Alcoholic Beveral!es. Neither party shall use or consume any beverage containing alcohol or use or consume nny proscribed dIlIg while exercising his or her rights to physical custody of the child. 6. Medical Carerrreatment. Plaintiff's agree to immediately infonn Defendant as to any problem of medical care or treatment of the child. 7. }telocation. Upon the knowledge of a pending relocation or movement, temporary or pennanen\, of either party, that party must immediately infonn the other party of the new address and telephone number. 8. No Movement Out of State. Neither party shall remove the child from the commonwealth of Pennsylvnnia to nnother state except through a Court Order.
