PETITION FOR NAME CHANGE
1. Petitioner is Jorma Z. Werntges, a thirty-one year old adult male residing at
310 East Coover Street, Mechanicsburg, Cumberland County, Pennsylvania.
2. Petitioner was born on December 6, 1971, in Pittsburgh, Pennsylvania.
3. At birth, Petitioner was named Jorma Z. Feydo. At the time of his birth, his
mother's surname was "Feydo".
4. Petitioner's mother's maiden surname is "Lucas".
5. Approximately twenty (20) years ago, Petitioner's name was changed from Jorma
Z. Feydo to Jorma Z. Wemtges, because Petitioner's mother had remarried and whose surname
was then "Werntges".
6. Petitioner's mother is now divorced from Mr. Werntges and has assumed her
maiden name of Bernadette Lucas.
7. Petitioner desires to change his name from Jomia Z. Werntges to Jorma Z. Lucas.
8. Petitioner is not seeking a name change for any improper purpose.
9. Petitioner is the father of a daughter, Madeleine Nicole Wemtges, age 5, with a
date of birth of December 3, 1997.
10. By separate Petition filed this date, Petitioner is seeking a name change for his
daughter from Madeleine Nicole Wemtges to Madeleine Nicole Lucas.

WHEREFORE, Petitioner Jorma Z. Wemtges respectfully requests your Honorable Court
to grant the relief sought in this Petition and order a change of name for Jorma Z. Wemtges to
Jorma Z. Lucas.

p.m., in Courtroom No. 1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
