Petitioner is Jorma Z. Werntges, an adult male individual residing at 310 East Coover Street, Mechanicsburg, Cumberland County, Pennsylvania. 2. Petitioner is the natural Father and Guardian of Madeleine Nicole Werntges, born December 3, 1997 and currently five (5) years of age. 3. Madeleine Nicole Werntges resides with her natural Father and natural Mother, Lindsay Michele Kilmore at 310 East Coover Street, Mechanicsburg, Cumberland County, Pennsylvania. 4. Madeleine's Mother, Lindsay Michele Kilmore, concurs with the relief sought in this Petition as evidenced by the Affidavit attached hereto as Exhibit A and incorporated herein by reference thereto. 5. Concurrently with the filing of this Petition, Jorma Z. Werntges is filing a separate Petition to change his name from Jorma Z. Werntges to Jorma Z. Lucas. 6. Petitioner desires his daughter to share the same sumame as he will have. 7. Petitioner desires to change his name from Jorma Z. Werntges to Jorma Z. Lucas. 8.
