Knowing that de~th is as much a reality as birth, if the
time comes when 1, JOAN A. WARGO, can no longer take part in
decisions for my own future, including the decision as to whether
or not my life shall be prolOnged by life support systems, I
desire that the following statement stand as an expression of my
wishes while 1 am sti 11 of sound mind, memory and understl!\l'ding:

If the situation should arise in which there is no
reasonable expectation of my recovery from physical or
mental disabil i ty, I request that I be allowed to di.e
and not be kept alive by artificial means or "heroic
measures."
I do not fear death itself as much as the
indignities of deterioration, dependence and hopeless
pain. I, therefore, ask that any medication be
mercifully administered to me to alleviate suffering
even t,l\Ough this may hasten the moment of my death. I
further direct that any life support systems to which
my body may be at.tached, including but not limited to
the artificial provision of nourishment, shall be
disconnected. I further request that everything be
done to assure that my death occur in my home or in a
hospice setting where I can be near my loved ones in my
final moments.
