4, The Petltioner, Elizabeth D. Baker, is the spouse of the alleged Incompetent, David E,
Baker, who was admitted to the Hershey Medical Center following a brain stem hemorrhage on May 16,
1997 and has remained there a9 of the date of the filing of this Petition,
6, The next of kin of David E, Baker are: James Baker, brother, Bolling Sprlngs,
PennsylVllllIa and Elizabeth D, Baker, spouse, Bolling Springs, PennsylvlUlia.
6, The said David E, Baker has no Income as of this date,
7, Elizabeth D, Baker and David E. Baker are owners of a home sltWlted at 20 Walnut

8, Your Petitioner believes that David E. Baker Is Incapacitated IlIld In need oca Guardian
of his Person IlIld Estate because he is sutTerlng from severe iqjurles 88 a result of the brain stem
hemorrhage which has resulted In him being In a COIl1ll continuously since May 16, 1997 until the date of
the flIlng of this Petition,
