D' AGOSTINO is unable to make decisions regarding financial matters or health care/medical
decisions, she is mentally retarded and has been since birth.
14, Theresa M. D' Agostino a/k/a Tercsa M. D' Agostino is 84 years of age and suffers
from senile demensia and is unable to carc for her daughter. P A TRIC A ANN D' AGOSTINO,
15, A separate Pctition to Adjudicate Incapacitated and Appoint Guardian of the
Person/Estate was filed with the Court on Deccmber 8. 1997. A hearing on that Petition is sct for
January 26. 1997 before the Honorable Judgc Bayley.
16, The flmctionallimitations and physical and mental condition of PA TRIClA ANN
D' AGOSTINO are: not able to make compctent decisions as far as hcr wclfare is concerncd.
17, The following stcps havc bcen taken. in order to find Icss restrictive alternatives to the
