HomeMy WebLinkAbout03-0856IN RE: YU-PING TAN IN THE COURT OF COMMON PLEAS OF CUMBERLAND COUNTY, PENNSYLVANIA CIVIL ACTION - LAW PETITION FOR CHANGE OF NAME AND NOW comes Yu-Ping Tan, by her attorney, Samuel L. Andes, and petitions this court for a change of her name, pursuant to the law of Pennsylvania, and based upon the following: 1. Petitioner is Yu-Ping Tan, an adult individual who was born on 3 July 1974. The Petitioner's legal residence is 5263 Strathmore Drive in Mechanicsburg, Cumberland County, Pennsylvania. 2. During the five years immediately preceding the filing of this Petition, Petitioner's legal residence as been 5263 Strathmore Drive in Mechanicsburg, Pennsylvania. 3. There are no judgments, decrees, or other adverse rulings by any court or other judicial tribunal entered against the Petitioner. 4. Petitioner requests that her name be changed to Amanda Yu-Ping Tan. She requests this change for the following reasons: A. Although her legal name has been Yu-Ping Tan since her birth, she has, throughout her life, used the name Amanda Yu-Ping Tan. B. As a result of her use of the name, Amanda Yu-Ping Tan throughout her life, she is known to virtually everyone outside her immediate family by that name. C. As a result of her use of the name, Amanda Yu-Ping Tan throughout her life, all or more of her school records, licenses, registrations, and other official documents are in, and have been throughout her life in, the name Amanda Yu-Ping Tan. D. Petitioner is known, both professional and personally, by the name Amanda Yu-Ping Tan. E. Changing her legal name to Amanda Yu-Ping Tan will reduce or entirely avoid any confusion between that name by which she has been known all of her life and her legal name. 5. Petitioner is not married and is not the parent of any minor children. 6. Petitioner has never been convicted of any crime. 6. Petitioner owes no debts for which she is in default, and owes no taxes which she has failed to pay. Petitioner does not file this Petition in an effort to avoid her creditors or evade her debts and obligations. WHEREFORE, Petitioner prays this court to change her name to Amanda Yu-Ping Tan.
