PETITION FOR APPOINTMENT
OF SUCCESSOR CUSTODIAN
AND NOW, comes Petitioner, Caroline L. Gilliam, by her
attorney, Daniel K. Deardorff, Esquire, who avers as follows:
1. Petitioner, Caroline L. Gilliam, is an adult individual
who resides at 39 Wilson Street, Carlisle, Cumberland County,
Pennsylvania,
2. Petitioner is the natural mother of Kellie L. Gilliam,
age 9 years, date of birth being June 9, 1971, who resides with
Petitioner.
3. Petitioner is the Administratrix of the estate of Gary
F. Gilliam, deceased, the natural father of Kellie L. Gilliam.
A copy of the Letters of Administration are attached hereto
as Exhibit "A",
4. Prior to his death on May 16, 1980, Gary F, Gilliam

had created a gift^pursuant to the Uniform Gifts to Minors
Act, 20 Pa. C.S. ~530l et seq.
5. The custodian of the gift was also its donor, Gary F.
Gilliam.
6. The type of property given by said gift was an investmen
account No. 0103-2297867-6 with Investors Diversified Services,

Inc. (IDS), a mutual investment company.
7, The present value of said account is approximately
$2,000.00.
8. All parties in interest, being only Caroline L.
Gilliam, join in this petition and therefore notice of this
petition need not be given. See Pestcoe Estate, 26 Pa. D.C. 2d
643 (1962).
\ffiEREFORE, Petitioner prays that Caroline L. Gilliam
be appointed successor custodian for Kellie L. Gilliam, a
minor, under the provisions of the Uniform Gifts to Minors Act,
for Account No. 0103-2297867-6 with Investors Diversified
Services, Inc,
