charges and set forth its claim as follows:
Statement of Claim
I. The name of the municipality hy which this claim is filed is the Borough of
Camp Hill, a political subdivision of the Commonwealth of Pennsylvania with offices located
at 2145 Walnut Street, Camp Hill, Cumberland County, Pennsylvania.
2. The name or names of the owner or owners, or reputed owner or owners of the
property against which this claim is filed is Clarence J. and Marion L. Landt.
3. A description of the property against which this claim is filed is the house and
property known as and numhered 2203 Page Street, Camp Hill, CUIIlherland County,
Pennsylvania.
4. The claim for collection of these unpaid amounts is hased upon the Muuicipal
Claims Act, 53 P.S. ~ 7101, et seq.
5. All acts, conditions, events and things required to he done under and hy the
Borough of Camp Hill have heen done and performed in due and legal form so as to entitled

the Borough of Camp Hill to a lien for the payment of the amounts for which this claim is
filed.
6. The sewer rate was charged for sewer service furnished to the above-described
property connected to the Borough of Camp Hill sanitary sewer system during the period from
200 1 through 2002 (3 periods).
7. The amount of the charges due under this claim are as foHows:
a. sewer charge at $120 per year in $60 bi-annual payments $180.00
b. sewer penalty and interest 26.56
c. Attorneys' fees (Ordinance 968) 135.00
d. minus partial payment _ 12.00
TOTAL CLAIM
Additional delinquent sewer charges to be included subsequent to filing.
