Faller, hereby files its claim for sewer services against the real estate hereinafter described
and sets forth its claim as folhlW,S:
STATEMENT OF CLAIM

The name of the municipal)ty by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed are Gary H. Rockwell and Beverly A. Rockwell of 1120 Newville
Road, Carlisle, Pennsylvania.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1120 Newville Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16,1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the
amount of $4,700.00.

Interest is claimed on the above-mentioned amount from September 20, 2006.

BEGINNING at a point in the middle of the public road leading from Carlisle to
Newville, commonly known as Newville Road; thence in a southerly direction 207.5 fect
to an iron pin in the northern line of a 16 foot alley; thence along said alley castwudly SO
fect to an iron pin; thence in a northerly direction along lands formerly of Reuben Lintner
207.5 feet to a point in the middle of the afonllaid State Road; thence along the middle of
said road wCltwardly 50 feet to a point, the place of BEGINNING.
BEING Lot No. 13 according to the Plan of Lots of Edward R. Lay, as the same
is recorded in the Office of the Recorder of Deed I in and for Cumberland County in Plan
Book 2, Page 40.
BEING improved with a dwelling house known and numbered as 1120 Newville
Road, Carlisle. Cumberland County Tax Parcel No. 29-20-1792-017.

Cumberland County in Deed Book 157, Page 940. granted and conveyed unto J. Michael
Adler and Betty L. Adler. Richard E. Adler is includccl herein to convoy any iutcrcst
he lllay have acquired in the subject property by virtue of marriage to Betty L. Adler.
This deed is given punuant to terms of and in Atiafaction oflData11mcnt Sales
Agreement dated October 28, 1997 and recorded in the Office oCthe Recorder ofDceds
afol'Claid in Miscellaneous Book 560, Page 657.
