Commonwealth of Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy &
Faller, hereby files its claim for sewer services against the real estate hereinafter described
and sets forth its claim as follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed are Richard A. Stephens and Roberta A. Stephens of 1152 Newville
Road, Carlisle, Pennsylvania.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1152 Newville Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the
amount of $4,700.00.

Interest is claimed on the above-mentioned amount from September 21, 2006.

WITNESSETH that the said GRANTORS, for and in consideration of the sum of ONE
HUNDRED FIVE THOUSAND AND NO HUNDREDTHS ($105,000.00) DOLLARS, in
hand paid, the receipt whereof is hereby acknowledged, the said GRANTORS do hereby grant,
and convey to the said GRANTEES their heirs and assigns as tenants by the entireties:
ALL THAT CERTAIN tract of land with the improvements thereon erected, situate in North
Middleton Township, Cumberland County, Pennsylvania, bounded and described as follows:
BEING all of Lot No. 5 and 10 feet of the Eastern part of Lot No. 6, as shown on the Plan of
Lots known as Westminster Manor, said Plan of Lots being recorded in the Office of Recorder
of Deeds for Cumberland County, Pennsylvania, in Plan Book 3, Page 67. Said lots fronting 65
feet on the Newville Road and extending in depth for an equal width 182 feet to an alley.
BEING improved with a one and one-half story dwelling house known as 1152 Newville
Road, Carlisle.
BEING the same property which Miriam B. Myers Hull and Frank Hull, her husband by Deed
dated November 3, 1980, and recorded in The Office of Recorder of Deeds in and for
Cumberland County, Pennsylvania, in Deed Book E-29, page 654, granted and conveyed unto
James K. Myers and Elizabeth A. Myers, his wife, Grantors herein.
IN WITNESS WHEREOF, the said Grantor has hereunto set her hand and seal the day and
