Faller, hereby files its claim for sewer services against the real estate hereinafter described
and sets forth its claim as follows:

STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of qwner or owners or reputed owner or owners, of the property against
which this claim is filed 'isElke A.. Light, 3301 Rt. 9 South, Suite 8274, Rio Grande, NJ 08242.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit" A" with an address of 1122 Newville Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the
amount of $4,700.00.

Interest is claimed on the above-mentioned amount from September 20, 2006.

receipt whereof is hereby acknowledged, the said Grantor does hereby grant
and convey to the said Grantee, her heirs and assigns:
ALL TIiAT certain tract of land situated in North Middleton Township,
Cumberland County, Pennsylvania, bounded and described as follows:
BEGINNING at a point in the middle of the concrete road between Carlisle
and Newville; thence in a Southerly direction 2(]7 feet along property now or
fonnerly of Ray Eyster, and known as Lot 113, to an iron pin in the Northern
line of a 16 foot alley; thence at right angles along &aid alley West 55 feet loan
iron pin; thence at right angles along lands now or formerly of Edward R. Lay
and Effie K. Lay North 2(]7 feet to a point in the middle of the aforesaid road;
thence along the middie of &aid road East 55 feet to a point the place of
beginning.
Said lot herein desaibed being Lot No~ 12 and the eastern flve feet olLot No.
11 according to the plan of Lots laid out by Edward R. Lay on October 11, 1924,
said Plan of lots being recorded in the Office of the Recorder of Deeds in and
for the County of Cumberland in Plan Book No. 2, page 40.
BEING Tract No.1 of the property conveyed by Norma A Cocldey, by her
attorney in fact, Richard P. CockIey, to BIke A. Light, by deed dated August 1,
