AND NOW COMES the Petitioner, Robert J. Kanc, and petitions your Honorable
Court follows:

Petitioner is Robert J. Kanc, of 782 Greenspring Road, Newville, Cumberland
County, Pennsylvania 17241.
On February 12, 2002, Petitioner was in Allentown, Pennsylvania.
While in Allentown, a vehicle break-in of Petitioner's automobile occurred at
approximately 6:30 P.M. or thereafter.
The following items were taken from Petitioner vehicle as a result of the break-in:
A. Wallet
B. Credit cards
C. Gun permit
D. Social Security Card
E. Miscellaneous identification items and other personal items.
The vehicle break-in was reported to the Allentown, Pennsylvania Police
Department to (Complaint # 03-12411) and the Pennsylvania State Police (Incident
#M05-0874400).
Subsequent to the break-in several of Petitioner's credit cards have been used
fraudulently in excess of $3000.00 by an unknown perpetrator at several places of
business.
Petitioner is greatly concerned about the potential future improper use of his
identity.

8. Petitioner was born in New Brunwsick, NeW Jersey on September 22, 1950.

Petitioner has specific concerns that his birth record may be illegally obtained and
used to his detriment.

Petitioner has inquired as to the procedure for blocking any illegal access to his
birth record.
11. Petitioner desires to prevent any illegal access to his birth record.
WHEREFORE, Petitioner respectfully requests that your Honorable Court enter an
Order:
1. Permitting Petitioner to obtain one (1) copy of a birth certificate with
raised seal from the New Jersey State Registrar, signed by Joseph Komosinski; and mailed
to Robert J. Kanc at 782 Greenspring Road, Newville, PA 17241. A check is enclosed in
the amount of $4.00 for the raised seal birth certificate; and
2. Permitting Petitioner to obtain one (1) copy of a birth certificate with
raised seal from the New Brunswick Health Department signed by local Registrar,
Margherita A. Kaplan and mailed to Robert J. Kanc at 782 Greenspring Road, Newville,
PA 17241; and
3. Preventing access to or otherwise sealing his birth records for a period
of five (5) years or until further Order of Court.
