AND NOW, comes Lanny Josiah Dum, by and through Frey & Tiley, Attorneys at Law,
and makes the following Petition:
1, Petitioner is Lanny Josiah Dum, an adult individual, residing at 403 Juniper Street,
Carlisle, Cumberland County, Pennsylvnia,
2, Petitioner was born August 19, 1951, in the Borough of Carlisle, Cumberland
County, Pennsylvania,
3, At the time of birth, Petitioner was given the name Lanny Josiah Dum, A copy of
the birth certificate evidencing this fact is attached hereto and is incorproated herein as Exhibit "A",
4, Petitioner has resided continuously in Cumberland County since October, 1986,
5, There are no judgments or decrees of record against Petitioner,
6, Petitioner has found his name "Dum", although a common name locally, to be a
source of ridicule and an impedimeot to engaging in transactions with others, especially when
dealing with persons outside of the central Pennsylvania area,
7, Petitioner now desires to have his name changed to John Lanny Duke,
WHEREFORE, Petitioner requests your Honorable Court to enter an Order granting the
Petition to change his name from Lanny Josiah Dum to John Lanny Duke,
