AND NOW, this j,-J. day of tJ ~ , 1998, upon consideration
attached Custody Conciliation Report, it is ordered and directed as follows;

1. This Court's prior order of September 22, 1998, shall remain in effect subject to the
following modifications:
A. In addition to Mother's periods of temporary custody as set forth in
the prior Order, Mother shall also eqjoy temporary custody of Mandi
on every Wednesday from after school until Thursday morning.
Furthennore, on those days when Mother is off work Tuesday
evening, Mother may pick the child up after school on Tuesday and
return the child on Thursday, this provision to be no more than 2
times per month.
B. Mother shall have custody of the minor child Mandi on
Thanksgiving Day from 2:00 p,m, until 8:00 p.m.
C, Mother shall have custody of Mandi on Christmas Eve from 4;00
p.m. until Christmas Day at I 1:00 a.m,
2. The parties shall discuss between themselves affording Mother a period of time on
either New Year's Eve or New Year's Day to be with Mandi. In the event the
parties are unable to reach an agreement, counsel for the parties may contact the
Conciliator to conduct a Conciliation Conference over the telephone after which the
Conciliator can recommend an appropriate order to this Court,
3, The parties shall meet again for a Conciliation Conference on the 2151 day of
JanuUlY, 1999, at 8:30 a.m,

this or another Court.
4. Plaintiff has no information of a custody proceeding concerning the
children pending in a Court of this Commonwealth.
5. Plaintiff does not know of a person not a party to the proceedings who has
physical custody of the children or claims to have custody or visitation
rights with respect to the children,
6. Each parent whose parental rights to the children has not been terminated
and the person who has physical custody of the children has been named as
parties to this action, There are no other persons who are known to have
or claim a right to custody or visitation of the children, so none will be
given notice of the pendency of this action and the right to intervene.
7. The mother requests this Court to grant an award of legal custody of Mandi
to her and to award her periods of partial custody as to J. Dean Robert,
with a direction to the mother, father, and both children, along with the
father's paramour, Cindy Holtry, to attend and successfully complete
counseling as deemed necessary, after evaluation and assessment.
