RENAUT, CIVIL ACTION - LAW Defendant CUSTODY ACTION CUSTODY COMPLAINT TO THE HONORABLE JUDGES OF SAID COURT: 1. The Plaintiff is Daryle E, Renaut residing at 314 E, Main Street, Mechanicsburg, Cumberland County, Pennsylvania 17055. 2. The Defendant is Cherie L. Renaut who resides at 407 N. Baltimore Avenue, Mt. Holly Springs, Cumberland County, Pennsylvania 17036. 3. Plaintiff seeks j oint legal custody and periods of partial custody of the following children: NAME PRESENT RESIDENCE DOB Alicia M. Renaut 407 N. Baltimore Ave 03/12/87 03/15/88 01/07/90 David Renaut 407 N. Baltimore Ave Christopher A. Renaut 407 N. Baltimore Ave David was born out of wedlock, the other children were not born out of wedlock. The children are presently in the physical custody of Cherie L. Renaut, who resides at 407 N. Baltimore Avenue, Mt. Holly Springs, Pennsylvania. - ~ DARYLE E. Jt: AND NCJol, th~s 1-- consideration of the attached and directed as follows: day of cJch 6CA.. , 1998, upon Custody Conciliation Report, it is ordered 1. The Father, Daryle E. Renaut, and the Mother, Cherie L. Renaut, shall have shared legal custody of David Renaut, born March 12, 1987, Christopher A. Renaut, born March 15, 1988, and Alicia M. Renaut, born January 7, 1990. Each parent shall have an equal right, to be exercised jointly with the other parent, to make all major non-emergency decisions affecting the Children's general well-being including, but not limited to, all decisions regarding their health, education and religion. Pursuant to the terms of this paragraph, each parent shall be entitled to all records and information pertaining to the Children including, but not limited to, school and medical records and information. To the extent one parent has possession of any such records or information that parent shall be required to share the same, or copies thereof, with the other parent within such reasonable time as to make the records and information of reasonable use to the other parent. 2. The Mother shall have primary physical custody of the Children. 3. The Father shall have partial physical custody of the Children on alternating weekends, beginning October 9, 1998, from Friday at 4:30 p.m. until Sunday at 7:00 p.m., and at such additional times as arranged by agreement of the parties when the Father is not working. 4. The Father shall also have partial physical custody of the Children for up to 4 weeks during each year, (to be taken non-consecutively unless otherwise agreed by the parties) upon providing 30 days advance notice to the Mother. The Father's periods of custody under this paragraph shall be scheduled during the Children's time off from school. 5. The parties shall share or alternate having custody of the Children during holidays as follows: A. Christmas: In every year, the Father shall have custody of the Children on Christmas Eve from 7:00 p.m. until 10:00 p.m. and from Christmas Day at 2:00 p.m. until December 26 at 7:00 p.m. The Father may have additional periods of custody with the Children over their Christmas school break when the Father is not working as arranged by agreement of the parties. The Mother shall have custody of the Children over Christmas Eve and Christmas Day at all times not otherwise provided for the Father, B. 'lbankagiving: The Thanksgiving holiday shall run from the Wednesday before Thanksgiving at 4:30 p.m. through Thanksgiving Day at 7:00 p.m. The Father shall have custody of the Children over the Thanksgiving holiday in even numbered years and the Mother shall have custody of the Children over the the Thanksgiving holiday in odd numbered years. In the event the Father's regular weekend period of custody falls ilTll1ediately after the Thanksgiving holiday in years when the Father has custody of the Children for Thanksgiving, the period of custody shall run continuously. C. Easter: In every year, the Father shall have custody of the Children from the Saturday before Easter at 4:30 p.m. through Easter Sunday at 2:00 p.m. and the Mother shall have custody of the Children on Easter beginning at 2:00 p.m. D. Memorial Day/Labor Day: The period of holiday custody over Memorial Day and Labor Day shall run from 4:30 p.m. on the day before the holiday through 7:00 p.m. on the day of the holiday. The Mother shall have custody of the Children on Memorial Day in odd numbered years and on Labor Day in even nu~ered years. The Father shall have custody of the Children on Memorial Day in even numbered years and on Labor Day in odd numbered years. E. Father's Day/Mother's Day: The Father shall have custody of the Children every year on Father's Day and the Mother shall have custody of the Children every year on Mother's Day, each for a substantial period of time as arranged by mutual agreement of the parties. 6. The party relinquishing custody of the Children shall be responsible to provide, tr.ansportation to the other party's residence for the exchange of custody. 7. The Mother shall provide the Children's regular activity schedules to the Father on an ongoing basis. The Mother shall also provide the Father with the Children's social security numbers for insurance purposes. 8. This Order is entered pursuant to an agreement of the parties at a Custody Conciliation Conference. The parties may modify the provisions of this Order by mutual consent.
