PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
The petitioner, Leslie Gilbert HOllinger, Jr., by and
through his attorneys, Keefer Wood Allen & Rahal, LLP, respect-
fully represents:
1. Leslie Gilbert Hollinger, Jr., was born on February 23,
1946, in Dauphin County, Pennsylvania, to Mildred I. Hollinger
and Leslie Gilbert HOllinger, Sr.
2. The Petitioner desires to change his name so that his
birth certificate, social security card, and other identifying
documents bear the name of Lester Gilbert Hollinger.
3. The Petitioner and his wife, Kathryn C. Hollinger, have
resided at 1445 Brandton Road, Lower Allen Township, Cumberland
County, Pennsylvania, since November 15, 1995.
4. Prior to establishing their current address, the
Petitioner and his wife resided at 1731 Anna Street, New Cumber-
land, Cumberland County, Pennsylvania, for a period of time which
began at least five years preceding the filing of this Petition,
and which ended on November 14, 1995.
5. There arc no judgmcnto or dccrccs of like character of
record against the Petitioner or his wife, with the exception of
a municipal lien (docket no. 96-2684 MLD Term), filcd on or about
May 13, 1996, by Lower Allen Township against rcal estate and
improvements thereon that are owned by them, pertaining to a
tapping fee and related costs levied in the construction of a
sanitary sewer system by said Township. The petitioner and his
wife are making monthly payments to satisfy said municipal lien
and are not behind in their payments.
6. Kathryn C. Hollinger, wife of the Petitioner, consents
to the granting of the change of name requested by the Peti-
tioner, as evidenced by her written consent attached hereto as
Exhibit A.
WHEREFORE, the Petitioner requests the Court, pursuant to
the Act of December 16, 1982, P.L. l309, No. 295, ~2, 54 Pa.
Cons. stat. ~ 702, to:
a. Fix a hearing on this Petition;
b. Direct that notice be given of this Petition
and hearing; and
c. Enter a decree changing the name of the Peti-
tioner from Leslie Gilbert Hollinger, Jr., to
Lester Gilbert Hollinger.
