TO THE DEFENDANT: The above named plaintiff(s) ask judgment in this Court against you for $$ 3, 21 9. 8 5plus court costs upon the following claim:
At the special instance an~ .request. of. Defendant,
Plaintiff provided advertising ln lts publlcatlon known as
the "Yellow Pages". After applying all proper credits,
there remains a principal balance due of $1,990.00, as set
forth in the attached statement of account. As provided in
the attached advertisement contract, Plaintiff is also
entitled to interest on the above amount from 07/23/96 to
03/12/98 to the agreed rate of 18% per annum totalling
$585.88 and its attorneys fees in the amount of $643.97,
for a total of $3,219.85, which, despite demand by
Plaintiff, remains due. Plaintiff maintains a place of
business in Philadelphia establishing a proper basis for Other:
