AND NOW, comes your Petitioner, William Goodhart, by and through his attorneys,
Mancke, Wagner, Spreha & McQuillan, and files the following request for reimbursement:
1. Your Petitioner, William Goodhart, is the guardian and natural parent of Allison
Goodhart.
2. Allison Goodhart was involved in an automobile accident for which proceeds are in a
trust account.
3. As a result of the automobile accident, Allison suffered permanent debilitating injuries
for which transportation is necessary through a special equipped vehicle.
4. That vehicle, which is a 1997 Dodge Grand Caravan, has throughout the recent period
of time, experienced mechanical difficulties for which expenses have been incurred, as more
fully set forth herein by way of Exhibit A.
5. Invoices are attached hereto that show payments for which Petitioner seeks
reimbursement.
6. The Petitioner herein, as guardian, respectfully requests the Court to direct that the
trust account with Members First FCU be directed to reimburse the Petitioner herein.
WHEREFORE, Petitioner requests the Court to grant the relief as requested.

AND NOW, comes your Petitioner, William Goodhart, by and through his attorneys,

1. Your Petitioner, William Goodhart, is the natural parent and guardian of Allison
Goodhart.
2. Allison Goodhart was born April 23, 1996.
3. On or about October 9, 1997, the aforementioned minor was involved in a very serious
automobile accident for which she received monies from the insurance company.
4. A previous request in January of 2007 was granted because of mechanical difficulties
of the specially equipped vehicle utilized by the family, the 1997 Dodge Grand Caravan.
5. That vehicle is now experiencing mechanical difficulties and significant body damage
that has rendered the Caravan's value less than what the costs to repair.
6. Petitioner has the opportunity to purchase a used van that is specially equipped for his
daughter which would be a perfect replacement of the current van.
7. Petitioner requests the Court to grant his Petition to permit the purchase of said used
cause a reduction or a loss in the account.
WHEREFORE, Petitioner prays this Court to grant the relief as requested.

Coming into my senior year, I know there are a lot of important things I have to
