O~-J~/~k- MUNICIPAL LIEN MUNICIPAL LIEN ~K3NICIPA~L LIEN FOR SERVICES AND SEWER RATES AND NOW, comes Monroe Township Municipal Authority, of 1220 Boiling Springs Road, Mechanicsburg, Pennsylvania, by and through its Solicitors, James D. Bogar, Esquire, and Jennifer B. Hipp, Esquire, and files the following municipal claim against VIOLET KATHRYN LOOK, an adult individual, owning property located at 1289 High Street, Boiling Springs, Monroe Township, Cumberland County, Pennsylvania 17007, her last known mailing address being: 1289 High Street, Boiling Springs, Pennsylvania 17007. By virtue of the death of HAROLD MONROE LOOK, a joint-tenant in ownership, said date of death being June 4, 1981, VIOLET KATHRYN LOOK, as the surviving joint-tenant, became and is the sole owner of all that certain lot of ground, including any improvements thereon, being more fully described as follows: ALL THAT CERTAIN house and lot of ground situate in the village of Churchtown, Monroe Township, Cumberland County, Pennsylvania, bounded and described as follows, to wit: BOUNDED on the North by High Street; on the West by lands formerly of Rudolph Kryshu, now or formerly of Norman Eby; on the South by lands formerly of Rudolph Kryshu, now or formerly of Hobart Souders; and on the East by lot now or formerly of Harper Zell. HAVING a frontage on High Street of 44 feet, more or less, and extending in depth at an even width 154 feet, more or less. HAVING ERECTED THEREON a single two and one-half story stucco dwelling house, being known and numbered as 1289 High Street, Boiling Springs, Pennsylvania. BEING the same premises which John E. McGill and Helen C. McGill, husband and wife, by Deed dated February 11, 1977, and recorded February 15, 1977, in the Cumberland County Recorder of Deeds Office, in Deed Book UA", Volume 27, Page 205, granted and conveyed unto Violet Katherine Look and Harold Monroe Look. Tax Parcel No. 22-28-2401-084 Monroe Township Municipality Authority hereby files this Municipal Lien for Services and Sewer Rates levied and assessed from May 18, 2001 through and including March 18, 2003 (second calendar quarter 2001 through and including first calendar quarter 2003). Said Authority services and sewer rates were levied and assessed pursuant to Authority Resolution No. 02-04, same being known as a Resolution of the Board of Monroe Township Municipality Authority imposing user charges to be collected from the owner of each improved property served or to be served by the Authority's sewer system, in conjunction with all prior relevant Resolutions and Ordinances, including, but not necessarily limited to, Monroe Township Ordinance No. 3 of 1978 and Monroe Township Ordinance No. 99-6, as amended; and providing for payment and collection of such user charges. Attached hereto and -2- incorporated herein are the statements forwarded by Monroe Township Municipal Authority to Violet Kathryn Look for sewerage due to be treated from May 18, 2001 through March 18, 2003 (second calendar quarter 2001 through and including first calendar quarter 2003), which services total is in the amount of $1,665.51, which sum was duly assessed against Violet Kathryn Look, and the real property described herein from the date of said billing statements as attached hereto, and for which sum, with interest in the amount of ten (10%) percent, along with penalties and all costs, including attorneys' fees, a municipal lien is claimed against the above-described real property and premises in accordance with the Act of May 16, 1923, P.L. 207, ~7, as amended, 53 P.S. ~7101, et seq, as amended (Municipal Claims and Tax Liens).
