C,,",,'I 7<..,.., PETITION FOR NAME CHANGE OF BIN SHENG 1,1 PETITION FOR CHANGE OF NAME Petitioner, Bin Sheng Li, through his attorneys, Strokoff & Cowden, P.C., requests that this Court order a change of name for him, pursuant to 54 Pa,C.p.s. 5701 et sea, and in support thereof represents as follows: 1. Petitioner Bin Sheng Li is a resident of 4968 Erbs Bridge Road, Mechanicsburg, Pennsylvania 17055, and has resided at the following locations during the five years next preceding the filing of this Petition: 1640 MacIntosh Way, Hummelstown, Pennsylvania (October, 1994 to September, 1997); 1654 25th Avenue No.2, Oakland, California (July, 1993 to September, 1994); 2125 Cabrill Highway, Half Moon Bay, California (October, 1992 to June, 1993); 840 Van Ness, San Francisco, California (June, 1992 to September, 1992); and 1650 19th Avenue, San Francisco, California (May, 1990 to December, 1991) 2. Petitioner immigrated to the United States from China in about May, 1990. He was issued a certificate of naturalization on September 17, 1998 (No. 23662565). A copy of that certification is attached hereto as Exhibit "A." 3. Petitioner wishes to change his name to Simon Lee to reflect his new American heritage and to assume a name which is more easily understood by the valJt majority of members of the public who are not familiar with Chinese names or the Chinese language, 4. Petitioner does not have any minor children. 5. Petitioner has requested the Prothonotary to search its records to determine whether there are any judgments or decrees of record against him. Petitioner believes, and therefore avers, that there are no such jUdgments or decrees against him. A copy of the certification submitted to the Prothonotary is attached hereto as Exhibit "B." 6. Pursuant to Act 83 of 1998, Petitioner has attached hereto as Exhibit "C" a copy of a fingerprint card showing his fingerprints. The original fingerprint card has been submitted to the Prothonotary.
