AND NOW, this,~(° day of July, 2001, comes the above-named petitioner,
Vladimir Freml, by and through his attorney, Courtney L. Kishel, Esquire, and The Law
Offices of Richard C. Gaffney, who respectfully requests This Honorable Court to grant
this petition for change of name, pursuant to 54 Pa.C.S. §§701 and 702 and in support
thereof avers the following:
Petitioner, Vladimir Freml, is an adult individual residing at 313 South
York Street in Mechanicsburg, Cumberland County, Pennsylvania,
17055. He has resided there since August 2000.
Prior to August 2000, Petitioner resided with his family at 1303 Loisa
Lane in Mechanicsburg, Cumbefland County, Pennsylvania, 17055.
Petitioner and his family resided there from April 1999 to August 2000.
Prior to that, Petitioner lived at 147 East Main Street in Mechanicsburg,
Cumberland County, Pennsylvania, 17055.
Petition is the son of Stjepan and Mira Freml, who currently reside at
with him at 313 South York Street in Mechanicsburg, Cumberland
County, Pennsylvania, 17055.
Petitioner has been a bona fide residem of the Commonwealth of
Pennsylvania for at least six (6) months prior to filing this petition.
Petitioner's father, Stjepan Freml, was bom January 1, 1944, in
Matijevidi, Croatia, under the name Stjepan Zvonimir Fr6mml. (See
Exhibit A).
Following the end of World War II, the occupying Communist
Government in Yugoslavia fomed Mr. Freml to change his name from
Stjepan Zvonimir Fr6mml, to Stjepan Freml. (See Exhibit A).
On July 1, 1972, Petitioner's father married Mira ~erbula, Petitioner's
mother, in Bosanski-Herzegovina. (See Exhibit A).
Petitioner was bom December 27, 1977 in Yugoslavia. (See Exhibit
B).
On May 8, 1997, Petitioner became a permanent residence of the
United States. (See Exhibit C).

On October 25, 2000, the Croatian Government granted Stjepan
Freml's petition to change his name from Stjepan Freml, to Stjepan
Fr6mml, which reflects the family's original name. (See Exhibit D).
11. Petitioner hopes to obtain United States citizenship in October 2002.

Petitioner wishes to obtain citizenship under the family's original
name, rather than the name given by the Communist Government,
because it reflects his family heritage.

Petitioner does not have any children.

Petitioner does not have any liens pending against him other than a car
loan he has received through Belco Credit Union.
15.
Petitioner is not attempting to avoid any civil or criminal liabilities, nor
is Petitioner filing this petition for any other fraudulent purpose.
WHEREFORE, for the foregoing reasons, Petitioner, Vladimir Freml,
respectfully requests this Honorable Court to grant this petition to change his name from
Vladimir Freml to Vladimir Fr6mml.

Odobrava se Stjepanu Freml iz SAD, posljednje prebivali~te u RH Ru-
jeva= iA, roden 01.01.1944. godine u Matijevi~i, Dvor, otac Ilija Freml,
majka Sarafina Freml r. Kujund~id, promjena prezimena iz Fr@ml u "Fr~mmt".
Po pravomodnosti rje~enja nala~e se nadle~nom mati~aru izvr~enje
upravne radnje naknadnog upisa odobrene promjene prezimena u dr~avnim
maticama.

Stjepan Fremt iz SAD, obratio se ovom Uredu putem Veleposlanstva
RH u SAD, Washington, Odjel za konzularne poslove, kojom tra~i promjenu
prezimena iz Freml u "Fr~mml".
U zamolbi navodi da ~eli promijeniti prezime u Fr~mml jet je to o-
biteljsko prezime, a izmijenjeno je poslije drugoga svjetskog rata na-
kon smrti njegovog oca Ilije Fr~mml, koji je poginuo 24.12.1944. godi-
ne u selu Struga, Dvor. Isti~e da je prezime tada promijenjeno u fonets-
ki oblik Freml koje je kori~teno sve do sada. Prila~e vjen~ani list svo-
jih roditelja iz kojeg je vidljivo da je prezime ~enika i njegovog oca
Fr~mml. Slijedom navedenog ~eli vratiti obiteljsko prezime Fr~mml.
Obavijest o promjeni prezimena objavljena jena oglasnoj Dlo~i ina
n3u nije bilo primjedbi.
U provedenom postupku, utvrdeno je da nema zapreka iz ~lanka 8, 9.
i 10. stavak 3. Zakona o osobnom imenu, te je primjenom ~lanka 6. stavak
1. istog Zakona, rije~eno kao u izreci.
Upravna pristojba na ovo rje~en]e napladena je u iznosu od 70,OOKn
upravnih biljega po Tbr. 1. i 2. Zakona o upravnim pristojbama /"Narod-
ne novine", broj: 8/96).
UPUTA U PRAVNOM LIJEKU:
Protiv ovog rjeDenja mo~e se izjaviti ~alba u roku 15 dana od dana
primitka rje~enja Ministarstvu pravosuda, uprave i lokalne samouprave,,
Zagreb, Republike Austrije 16, kao drugostupanjskom tijelu.

Dvor, in the subject of surname change, on the basis of the Article 6, Item 3 of Personal
Name Law ("Narodue novine", number 69/92), has made
The Decision
It is allowed to Stjepan Freml from the USA, with last residence in the Republic
of Croatia in Rujevac IA, bom on January 01, 1944, at Mafijeviti, Dvor; father Ilija
Freml, mother Sarafina Freml nee. Kujundlfi~, the change of sm'name from Freml into
"Fr0mml'.
By the validation of the decision the authorized registrar is ordered to perform the
administrative action of subsequent registration of the allowed surname change in the
state register of births, marriages and deaths.
Explanation
Stjepan Freml from the USA has contacted this Office by the Embassy of the
Republic of Croatia in Washington Consular Affairs Depm [ll~ent, with which he requires
the surname change from Freml into "Fr6mml".
In the request he states that he wishes to change the surname to FrOmml, since it
is the family's surname, and had been changed after the World War Two following the
death of his father Ilija FrOmml, who was killed on December 24, 1944 at village of
Struga, Dvor. He emphasizes that the surname had been changed to phonetic form of
Freml at that occasion, and was used until now. He encloses the certificate of marriage of
his parents', which makes visible that the name oftbe groom and his father is Fr6mml. In
sequence to the stated, he wishes to regain his family's surname of Fr6mml.
The notice on surname change has been posted on the notice board and no
objections to it have been raised.
In the procedure which was conducted, it has been established that no obstacles
from Articles 8, 9 and 10 item 3 of the Personal Name Law exist, and therefore by
application of the Article 6, item 1 of the Law, the decision has been made as in the
proposition.
LEGAL REDRESS ADVICE: ·
The complaint can be filed against this decision within the course of 15 days of
the day of the receipt of it, to the legal body of second instance, the Ministry of Justice,
Administration and Local Self-Governing, in Zagreb, 16 Republike Austrije Street.
