2000. Prior to that, Petitioner resided at 147 East Main Street in
Mechanicsburg, Cumberland County, Pennsylvania, 17055.
Petitioner has been a bona fide residem of the Commonwealth of
Pennsylvania for at least six (6) months prior to filing this petition.
Petitioner was bom November 10, 1946, in Kosna, and given the name
Mira ~erbula. (See Exhibit A).
Petitioner's husband was bom January 1, 1944, in Matijevi6i, Croatia,
under the name Stjepan Zvonimir Fr6mml. (See Exhibit B).
Following the end of World War II, the occupying Communist
Government in Yugoslavia forced Mr. Freml to change his name from
Stjepan Zvonimir Fr6mml, to Stjepan Freml. (See Exhibit B).
On July 1, 1972, Petitioner married Stjepan Freml in Bosanski-
Herzegovina and took her husband's last name of"Freml".
On May 8, 1997, Petitioner and her husband became a permanent
residence of the United States. (See Exhibit C).
On October 25, 2000, the Croatian Government granted Petitioner's
husband's request to change his name from Stjepan Freml, to the
original spelling of Stjepan Fr6mml. (See Exhibit D).
Petitioner hopes to obtain United States citizenship in October, 2002.
Petitioner wishes to obtain citizenship under her husband's original
name, rather than the name given to him by the communist
government, because it reflects her husband's family heritage.

Petitioner has one son, Vladimir Freml, who has also filed a petition to
change his name to the original family spelling.

Petitioner does not have any liens pending against her.

Petitioner is not attempting to avoid civil or criminal liabilities, nor is
she filing this petition for another other fraudulent purpose.
WItEREFORE, for the foregoing reasons, Petitioner, Mira Freml, respectfully
requests This Honorable Court to grant this petitioner to change her name from Mira
Freml, to Mira ~erbula FrOmml.

Stjepan Freml iz SAD, obratio se ovom Uredu putem Veleposlanstva
RH u SAD, Washington, Odjel za konzularne poslove, kojom tra~i promjenu
prezimena iz Freml u "Fr~mml".
U zamolbi navodi da ~eli promijeniti prezime u Fr~mml jer je to o-
biteljsko prezime, a izmijenjeno je poslije drugoga svjetskog rata na-
kon smrti njegovog oca Ilije Fr~mml, koji je poginuo 24.12.1944· godi-
ne u selu Struga, Dvor. Isti~e da je prezime tada promijenjeno u fonets-
ki oblik Freml koje je kori~teno sve do sada. Prila~e vjen~ani list svo-
jih roditelja iz kojeg je vidljivo da je prezime 2enika i njegovog oca
Fr~mml. Slijedom navedenog ~eli vratiti obiteljsko prezime Fr~mml.
Obavijest o promjeni prezimena objavljena jena oglasnoj plo~i ina
nju nije bilo primjedbi.
U provedenom postupku, utvrdeno je da nema zapreka iz ~lanka 8, 9.
i 10. stavak 3. Zakona o osobnom imenu, te je primjenom ~lanka 6. stavak
t. istog Zakona, rije~eno kao u izrec±.
Upravna pristojba na ovo rje~en]e napladena je u iznosu od 70,00Kn
upravnih bi!jega po Tbr. 1. i 2. Zakona o upravnim pristojbama /"Narod-
ne novine", broj: 8/96).
UPUTA U PRAVNOM LIJEKU:
Protiv ovog rje~enja mo~e se izjaviti ~alba u roku 15 dana od dana
primitka rje~enja Ministarstvu pravosuda, uprave i lokalne samouprave,
Zagreb, Republike Austrije 16, kao druqostupanjskom tijelu.

Freml from the USA, with the last residence in the Republic of Croatia at Rujevac lA,
Dvor, in the subject of surname change, on the basis oftbe Article 6, Item 3 of Personal
Name Law ("Narodne novine", number 69/92), has made
The Decision
It is allowed to Stjcpan Freml from the USA, with last residence in the Republic
of Croatia in Rujevac lA, born on Januax3t 01, 1944, at Matijevi6i, Dvor; father Ilija
Freml, mother Samfma Freml nee. Kujund~i6, the change of surname from Freml into
"FrOmml".
By the validation of the decision the authorized registrar is ordered to perforra the
administrative action of subsequent registration of the allowed surname change in the
state register of births, marriages and deaths.
Explanation
Stjepan Freml from the USA has contacted this Office by the Embassy of the
Republic of Croatia in Washington Consular Affairs Department, with which he requires
the surname change from Freml into "FvOmml".
In the request he states that he wishes to change the surname to Fr6mml, since it
is the family's surname, and had been changed after the World War Two following the
death of his father Ilija FrOmml, who was killed on December 24, 1944 at village of
Slxuga, Dvor. He emphasizes that the surname had been changed to phonetic form of
Freml at that occasion, and was used until now. He encloses the certificate of marriage of
his parents', which makes visible that the name of the groom and his father is Fr6mml. In
sequence to the stated, he wishes to regain his family's surname of FrOmml.
The notice on surname change has been posted on the notice board and no
objections to it have been raised.
In the procedure which was conducted, it has been established that no obstacles
from Articles 8, 9 and 10 item 3 of the Personal Name Law exist, and therefore by
application of the Article 6, item 1 of the Law, the decision has been made as in the
proposition.
LEGAL REDRESS ADVICE:
The complaint can be filed against this decision within the course of 15 days of
the day of the receipt of it, to the legal body of second instance, the Ministry of Justice,
Administration and Local Self-Governing, in Zagreb, 16 Republike Austrije Street.
