The Petitioner MICIIAEL PATRICK BERGSTROM, through his undersigned
counsel, Richard L. Webber, Jr, request that this COllrt order of change of name lor him,
pursuant to 54 C S.A Section 701 ct seq., and in support thcreofrcpresents as follows:
I. Petitioner is Michael Patrick Bcrgstrom
2. Petitioner prescntly residcs, and has resided at 50 Pine Knob Road, Ncwville, Upper
Mifllin Township, Cumberland County, Pennsylvania, during the ten years preceding this
filing of this Petition.
3. Petitioncr is employed at IBM, 5267 Simpson Ferry Road, Mcchanicsburg, PA 17055.
4. Petitioner is 22 years of age (DOB 9/25/75).
5, Petitioner desires to change his name for the following rcasons:
A Petitioner has used the surname "Wolfe" since he was several years old;
B. Petitioner's mother, Ann Wolfe, has resided with her husband Robert Wolfe
since 1977 and has been married to him since 1979;
C. Petitioner has resided with his mother, Ann Wolfe since birth and with Robert
Wolfe since 1977;
D, Petitioner has only scen his natural father Jamcs Bergstrom on approximately 7
occasions since birth.
E. Petitioner's natural father resides in Calif(,rnia,
F. Petitioncr has a c10sc relationship with his half-brother, LlIcas Wolfe. who has

G. Petitioner believes that the sllrname "Wolfe" reflects his true idcntity
6. There are no judgments, lax obligations or dccrees of like character of record against
Petitioner.
WHEREFORE, Petitioner requests the Court, pursuant to the Act of December
16,1982, P.L. 1309, No. 295, Section 2, 54 C.S.A. Section 702, to:
a. Schedule a hearing on this Petition;
