1992-512 dated 06/04/92, by the Court of Ccmron Pleas of Cumberland County. 1'he
P.F.A. No. 98-6303 Civil Term was issued by the Honorable Judge George E Hoffer on
the 5Th day of November, 1998.
PROBABLE CAUSE, The Defendant Anthony Quinn Stackfield did threaten his wife inside
the hare at Lot 117 Big Springs Terrace. He was swinging a long handled pruning
snips and telling her that she was digging her own grave. He followed her into the
bathroom and pushed her into the tub. All was done in violation of the P.F.A. order.

YOU HAVE BEEN SUED IN COURT. If you wish to defend against the claims set forth in the

Protection From Abuse, the court hereby enters the following Temporary Order:
[Xl 2. Defendant is excluded from the residence at 117 Big Spring Terrace, Newville, P A 17241 or any other
permanent or temporary residence where Plaintiff may live. Plaintiff is granted exclusive possession
of the residence. Defendant shall have no right or privilege to enter or be present on the premises,
[X] 3. Except for such contact with the minor children as my be permitted under Paragraph 5 of this Order,
Defendant is prohibited from having ANY CONTACT with Plaintiff at any location, including but not
limited to any contact at Plaintiffs school, business, or place of employment.
[Xl 4. Except for such contact with the minor children as may be permitted under Paragraph 5 of this Order,
Defendant shall not contact Plaintiff by telephone or by any other means, including through third
persons.

The local law enforcement agency in the jurisdiction where the children are located shall ensure that the

location where a violation of this order occurs OR where the defendant may be located. If defendant violates

An arrest for violation of this Order may be made without warrant, based solely on probable cause, whether
or not the violation is committed in the presence of law enforcement.

used during the violation of this Order OR during prior incidents of abuse. Weapons must forthwith be
delivered to the Sheriffs office of the county which issued this Order, which office shall maintain possession

11. The following other minor child prcsently livcs with Plaintiff: Stcphen Dusean Robinson, age 10.
Plaintiff is Stephen's mother.
12. The facts of the most recent incident of abuse are as follows:
On October 31, 1998, Plaintiff was walking up to the home where her babysitter was caring for
her children. Defendant was outside waiting for her. He walked up to her, raising his voice,
and was upset about his child's Halloween costume. An altercation arose wherein Dcfendant
pushed Plaintiff twice, once resulting in Plaintiff falling to the ground. When Plaintiff got up,
she crossed the street, where Defendant followed her. Defendant then pulled a machete from
his coat and began swinging it toward Plaintiff, putting her in fear of imminent serious bodily
injury, Defendant was restrained by one of his friends, while the baby siller called the police.
When the police came, they issued Defcndant a citation, and told him to stay away from Plain-
tiff. Defendant then followed Plaintiff for two hours, while Plaintiff was triek-or-treating with
her children. Defendant would follow Plaintiff, then go away, then come back, putting Plaintiff
in fear of imminent serious bodily injury.
13. Defendant has abused Plaintiff on other occasions. In or about August of 1998, Defendant and Plaintiff
were engaged in an altercation, wherein Defendant was chasing Plaintiff around a football field.
Defendant was allempting to take Plaintiffs purse, and Plaintiff resisted. Defendant grabbed
Plaintiffs purse, and Plaintiff attempted to get in her car to leave. Defendant then slammed
Plaintiffs leg in her car door, causing bodily injury which necessitated treatment at the Carlisle
Hospital emergency room.
In approximately September of 1997, Defendant put a gun to Plaintiffs head and claimed it was
unloaded. Defendant did have bullets for the gun, and told Plaintiff he "had a bullet with her
name on it". Plaintiff was in fear of imminent serious bodily injury.
14. Defendant has used both a handgun and a machete to abuse Plaintiff.
15. The State Police substation which covers Newville should be made aware of any Order in this matter.
16. There is an immediate and present danger of further abuse from the Defendant.

4. Defendant shall not contact the Plaintiff by telcphone or by any othcr means, including through third
persons, with the cxccption of Plaintiffs allomcys.

CHIEF COMPLAINT: Ankle pain
HISTORY OF PRESENT ILLNESS: Twenty-elght-year-old female said she closed her right ankle in a
car door. She IS ambulatory, but IS uncomfortable.
PHYSICAL EXAMINATION: Alert, oriented female who is somewhat uncomfortable.
Blood pressure 142/100. Respirations 20. Pulse 120. Temperature 36.50. She has a normal appear-
ing ankle on visual inspection. There is no obvious bony deformity. Normal dorsalis pedis pulse. She
has pain near the insertion of the Achilles tendon, but medial to that in the distal aspect of the gastroc
musculature. She seems to have full range of molion, but it is somewhat uncomfortable for her.

DATABASE: X-ray of ankle per emergency department MD interpretation reveals no fracture or dislo-
cation.

PLAN: Ice, ibuprofen and follow-up with her family doctor as needed.

4. I understand that many of the physicians on the staff of this hospit.al, including
the attending physician (s) named above, are not employees or agents of the hospital but,
rather, are independent contractors who have been granted the privilege of using its
facilities for the care and treatment of their patients. Further, I realize that among those
who attend patients at this hospital are medical, nursing, and other health care personnel
in training who, unless requested otherwise, may be present during patient care as a part of
their education. Still or motion pictures and closed circuit television monitoring of
patient care also may be used for educational purposes or for documentation of the clinical
course unless a patient expressly requests otherwise.
5. I release CARLISLE HOSPITAL from all responsibility for all articles which I am
retaining or will have with me during my stay at the hospital. I understand this includes
clothing, bridgework, false teeth, eyeglasses, jewelry, money, radio, razor or any other item
kept in my possession. I understand I may deposit valuables in a safe provided by the
hospital; only if this is done will the hospital assume any responsibility for the
safekeeping.
