The father of the child is MARVIN D. CAMPBELL. currently residing at the State
Correclionallnstitution at Camp Hill. He is married.
4. The relationship of the Plaintiff to the child is that of natural mother. The
Plaintiff currently resides with the following persons: CHANCELLOR D. D. CAMPBELL, her
son.
5. The relationship of the Defendant to the child is that of natural father. The
Defendant currently resides with his fellow prisoners at the State Correctionallnstilution at Camp
Hill, Pennsylvania.
6. Plaintiff has not participated as a party or witness, or in any other capacity
in other litigation concerning the custody of the child in this or another jurisdiction.
7. The Plaintiff has no information of a custody proceeding concerning the
child pending in a court of the Commonwealth.
8. The Plaintiff does not know of a person not a party to the proceedings who
has physical custody of the child or claims to have custody or visitation rights with respect to the
child.
9. The best interest and permanent welfare of the child will be served by
granting the relief requested because the child has lived with his mother all of his life and she
has cared for all of his needs, during a time when Defendant has been incarcerated.
10. Each parent whose parental rights to the child have not been terminated
and the person who has physical custody of the child have been named as parties to this action,
No other persons are known to have or claim a right to custody or visitation of the child.

1915.3-8, the undersigned custody Conciliator submits the following report:
1. The pertinent information concerning the Child who is the
subject of this litigation is as follows:

2, A Conciliation Conference was held on December 16, 1998, with
the following individuals in attendance: The Mother, Monique Y.
Doleman-campbell, with her counsel, James D. Flower, Jr" Esquire. The
Father, Marvin D. Campbell, is currently incarcerated in the State
Correctional Institution in Camp Hill and did not attend the Conference.
It was unclear whether the Father received service of the Notice of the
Conciliation Conference and efforts by the Mother's counsel to contact the
Father by telephone at the time of the Conference were unsuccessful.
3. Based upon representations made by the Mother at the
Conference that the Father was incarcerated prior to the Child's birth in
1994, that the Child has had only very limited contact with the Father
during the Father's incarceration, that the earliest that the Father could
be released from prison would be July 1999, and that the Mother is only
able to reach the Father by telephone in the event of an emergency, the
Conciliator recommends an Order in the form as attached. The Order
provides a thirty (30) clay period in which the Father may appeal entry of
the Order if the Father objects to the custody provisions.
