I, I'lainlifT is Tony Monnell, lid/blaT & M Automotive, a Pennsylvania sole
proprietorship with his principal place ofbusincss locatcd atl412 Trindlc Road, Carlisle,
Cumberland Counly, Pennsylvania
2, Defcndant, Don Rich, an adult individual, rcsides at 63 Colfax Road, Upper Darby,
Pennsylvania 19082,
3, On or about November 3, 1997, Defendants vehicle, a 1992 G20 Maroon Van,
VIN # IGBEG25K3N710638, was towed to T & M Automotive with transmission problems,
4, On or about November 3, 1997 T & M Automolive removed and replaced the
transmission from lhe vehicle; disassembled transmission; cleaned and resealed parts; installed
new friction's and steels; installed new sealing rings and 2-4 band; reassembled transmission;
installed new convertcr; lilled with fluids and road lested the vehicle,
5, Plaintiff performed all oflhe labor and supplied all of the necessary parts to make
the vehicle operable,
6, Plaintiff completed lhe work in a workmanlike manner.
7. On November 6, 1997, Defendant, Don Rich, signed an invoice in the amount of
$1,082.31, for the work performed and drove said vehicle from premiscs, (see attached
Exhibit A)
