The adcquaey of the considcration liJr all agreemcnts hercin contained is stipulated, confessed,

Each palty to the Agrcement acknowledgcs and declares that he or she,

counsel, understands that he or she has the right to counsel;

WAIVER OF CLAIMS AGAINST ESTATES: Exccpt as hercin otherwise providcd,

III. Other Provisions
I. This Ordcr is intcnded to constitutc a qualificd domcstic relations order within thc
mcaning ofscction 414(1') of the Internal Rcvcnue Codc of 1986, as amended and
section 206(d) of the Employec Retirement Income Security Act of 1974, as
amended, and shall be inte'1Jreted in a manner consistent with such intcntion.
2. The Court shall retain jurisdiction to amend this Order to the extent necessmy to
establish or maintain its status as a qualified domcstic rclations order.
3. It is rccognized that the Alternate Payee may eleet to commence receIVIng
bendits before the Partieipant retircs. I l' the Alternate Payce so rcqucsts, the
Parlicipant will cooperatc with the Alternatc Payee in substantiating a claim or
application to the Fund and shall provide any documentation or infonnation
reasonably nccessary to establish their eligibility for benefits,

distribution under the Plan, or (ii) thc later of (a) the date the Participant attains

III. Other Provisions
1. This Ordcr is intendcd to constitutc a qualitied domestic relations ordcr within thc
mcaning ofscction 414(p) ofthc Internal Rcvenuc Code of 1986, as amendcd and
section 206(d) of the Employec Retircmcnt Income Security Act of 1974, as
amended, and shall bc interprctcd in a manncr consistent with such intention.
2. Thc Court shall rctain jurisdiction to amcnd this Order to thc cxtent necessary to
establish or maintain its status as a qualified domestic relations order.
3. It is recognized that the Alternate Payee may elect to cOl11mcnee recelvll1g
bencfits before the Participant rctires, If the Altcrnate Payee so requests, the
Participant will cooperate with the Alternate Paycc in substantiating a claim or
application to thc Fund and shall providl~ any documentation or infon11ation
