They are the parents ~d custodi~s of Christyn Dunning, a minor, age 11, who was injured in a motor vehicle accident on July 20, 1997. 2. Christyn Dunning, a minor, currently resides with her parents, Eugene ~d Tamara Dunning, at 22 Ivy Terrace in Carlisle, CurnberI~d County, Pennsylvania 17013. 3. This petition is brought in Curnberl~d County and the accident also occurred in Curnberl~d County, Pennsylv~ia. 4. The accident occurred when the vehicle in which Christyn Dunning was a passenger, was struck by a vehicle driven by Kenneth L Compo 6. As a result of the aforesaid incident, Christyn Dunning sustained injuries to her abdomen as a result of being suspended by the waist belt. t\: 7. ChriStyIl Dunning was treated by the Carlisle Hospital Emergency Room and did receive treatment following the date of this accident from Carlisle Pediatrics on July 25,1997. 8. The following expenses have been incurred for treatment of said minor, all of which have been paid in full by the Dunning's State Farm Automobile Insurance Policy. Carlisle Hospital Carlisle Pediatrics Total Medical Expenses $ 60.00 48.58 $158.58 9. Counsel was retained upon a 35% contingent fee basis by Petitioner, but has agreed to reduce that fee to 25%. Additionally, counsel has incurred the following expenses: Carlisle Pediatrics Cumberl~d County Prothonotary (Filing Minor's Petition) Total Expenses $ 15.00 45.50 $ 60.50 10.
