PETITION FOR NAME CHANGE
AND NOW, comes Petitioner, Genivaldo Barboza Macuje, by and through his attorney,
Jacqueline M. Verney, Esquire, and in support of his Petition For Name Change avers the
following;
1. The Petitioner in this matter is Genivaldo Barboza Macuje, an adult individual,
(DaB 2/13/61), who currently resides at 1008 Northfield Drive, Carlisle,
Cumberland County, Pennsylvania, 17013.
2. The Petitioner has resided at the following addresses, throughout the five-year
period immediately prior to the filing of the Petition for Name Change.
1008 Northfield Drive, Carlisle, PA
June 1998-Present
3801 Conshohockin Ave., Philadelphia, PA
January 1993-June 1998
3. Petitioner wishes to change his name formally to Marcos Andre Enrique DeLano.
Mr. Macuje became a United States citizen in August, 1999 and at that time had the
opportunity to change his name. He did not have sufficient opportunity to think
about changing his name at that time. After thinking about it for a year, he has
decided that his present name is too difficult for otheTs to pronounce and would like
a more easily pronounced name while still maintaining his heritage.

4. If this Petition is approved, the Petitioner intends to use the name Marcos Andre
Enrique DeLano consistently, non-fraudulently, and exclusively.
5. It is not the purpose of this Petition to avoid any taxes or any other debts.
