AFFIDAVIT FOR MECHANIC'S AND MATERIALMAN'S LIEN
BEFORE ME, the undersigned authority, personally appeared Tony Lehew who
upon his oath, deposed and stated the following:
"1. My name is Tony Lehew. I am the Vice President of Discovery Quality
Cabling, Inc., hereinafter sometimes referred to as "Claimant." I am over 18 years of age,
have personal knowledge of the facts set forth below and am competent and authorized
to make this affidavit.
2. Claimant's business address is 1846 W. Division, Suite 306, Arlington,
Texas 76012.
3. Claimant furnished labor and materials for improvements to the real
property by installing cabling on the real property.
4. The real property sought to be charged with a lien by Claimant is described
as follows: The Radisson Suites Hotel located at 1150 Camp Hill Bypass, Camp Hill,
Pennsylvania 17011.
5. Claimant furnished labor and materials for improvements to the real
property under a contract by and between Claimant and Self & Associates, Inc. whose
address is 1114 South Airport Circle, Suite 130, Euless, Texas 76040.
6. Claimant served notice to Self & Associates, Inc. on June 22, 2000.
7. Work was last performed on the project on or about April 16, 2000.
8. A detailed description of the work completed is attached hereto as Exhibit
"A".
9. After allowing all just credits, offsets, and payments, the amount of
$3,434.00 remains unpaid and is due and owing to Claimant under its contract with Self &
Associates, Inc. whose address is 1114 South Airport Circle, Suite 130, Euless, Texas
76040, and Claimant claims a lien on said property and improvements under the
provisions of Pennsylvania law."
