for unpaid water and sewer services rendered from January, 1997 to September, 2000. Said
services were rendered and charged pursuant to applicable rate resolutions of the Middlesex
Township Municipal Authority.
Attached hereto is a statement of the kind and character of the services so rendered and
the prices charged therefor, said statement being a true and correct copy of the Owner's account
with the Claimant, which sum of $1,104.65 for the above period was duly assessed against the
above Owner and premises beginning September, 1997 and for which sum, which includes
interest, costs and penalties, a lien is claimed against the above premises in accordance with the
Municipal Authorities Act of 1945, May 2, 1945 P,L. 382, 53 P.S. 99301 et seq., as
