Klucker, by her attorney, Harold S. Irwin, III, Esquire, and presents this petition for change of name, representing as follows: 1. Petitioner is Terry E. Klucker, who resides at 2 W Willow Lane, Country Manor, Carlisle, Cumberland County, Pennsylvania 17013. 2. Petitioner is an forty-six (46) year old adult female, whose date of birth is April 15, 1954. 3. Petitioner has resided in the Commonwealth of Pennsylvania since the 1970's and in Cumberland County since 1986. 4. On January 21, 2000, petitioner filed a complaint in divorce against her then current husband, James L. Klucker. Prior to the entry of a decree in divorce, on March 30, 2000, James L. Klucker was killed in an automobile accident. 5. Petitioner wishes to adopt the name Victoria Brianne Zeiss for the following reasons: A.
