1. Plaintiff is CSH Industries located at 2500 Old Gettysburg
Road, Camp Hill, PA 17011.
2. Defendant is E-Z Kleen located at 329 E. Orange Street,
Shippensburg, PA 17257.
3. On June 1, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 037699, The unpaid balance owing to
CSH Industries for those products is $665.26.
4. On June 1, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 037765, The unpaid balance owing to
CSH Industries for those products is $8,342,20.
5. On June 18, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 037733. The unpaid balance owing to
CSH Industries for those products is $546.96.
6. On August 17, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 038710. The unpaid balance owing to
CSH Industries for those products is $212.00.
7. On August 18, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 038733. The unpaid balance owing to
CSH Industries for those products is $345.83.

8. On October 1, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No, 039495. The unpaid balance owing to
CSH Industries for those products is $175.96.
9, On October 29, 1999 CSH Industries sold to E-Z Kleen various
commercial products at invoice No. 039793. The unpaid balance owing to
CSH Industries for those products is $389.55.
10. On November 30, 1999 CSH Industries sold to E-Z Kleen
various commercial products at invoice No. 001626-00.
The unpaid
balance owing to CSH Industries for those products is $442.44.
11. CSH Industries has requested that E-Z Kleen pay the
aforesaid balances set forth above, however, E-Z Kleen has not paid the
same.
12, The total amount owing to CSH Industries by E-Z Kleen is
$11,120.20,
WHEREFORE, CSH Industries requests judgment against E-Z Kleen in
the amount of $11,120.20 plus whatever interest or late charges may
accrue up until the date of judgment.
