ANDERSON & SACKS the stun of FIFTY-FIVE THOUSAND SIX HUNDRED
FOURTEEN AND EIGHTY-FOUR HUNDREDTHS DOLLARS ($55,614.84) in lawful
money of the United States of America.
AND FURTHER, I do hereby authorize and empower any attorney of any Court
of record of Pennsylvania or elsewhere to appear for and enter judgment against me for
the above stun, with or without declaration, with costs of suit, release of errors, without
stay of execution, and with fifteen percent (15%) added for collection fees; and I also
waive the right of inquisition on any real estate that may be levied upon to collect this
Note, and do hereby voluntarily condemn the same, and authorize the Prothonotary to
enter upon the Writ of Execution said voluntary condemnation; and I further agree that
said estate may be sold on a Writ of Execution, and I hereby waive and release all relief
from any and all appraisement, stay or exemption laws of any state now in force or
hereafter to be passed,

A representative of SMIGEL, ANDERSON & SACKS has explained to me that
the Note contains wording that would pennit them to enter judgment against me at the
Courthouse, whether or not said Note is in default, without Notice to me and without
offering me an opportunity to defend against the entry of Judgment, and that the
Judgment may be collected by any legal means.

my rights to resist the entry of Judgment against me at the Courthouse and I am
