Indiana, Pennsylvania on March 25, 1979.

Petitioner is an single adult individual who is the age of Twenty One (21) years.

There are no judgments of decrees or liens of like character against the petitioner in the
name of Kathy Jo Rhoades.
WHEREFORE, petitioner respectfully request s your Honorable Court to enter an
