The Plaintiffi'Claimant is James Musico, a sole proprietor with a place of business located at 400 Black Hawk Drive, Schuylkill County, Permsylvania 17922. 2. The Plaintiffi'Claimant files the within Mechanics' Lien Claim as a subcontractor as defined in the Mechanics' Lien Law of 1963,49 P.S. 1101 et seq. 3. The Defendant(s)/Owner(s) are West Shore Plaza, Inc., with an address at 1200 Market Street, West Shore Plaza, Lemoyne, Cumberland County, Permsylvania 17043. 4. The date of completion of the Plaintiffs/Claimant's work on the Premises was August 24, 2000. 6. The Plaintiff/Claimant performed work on the Premises pursuant to a contract with Appalachian Equipment of 7059 Linglestown Road, Harrisburg, Dauphin County, Pennsylvania 17112, which contracted with the Defendant(s)/Owner(s). 7. The work performed by the Plaintifti'Claimant on the Premises consisted of supplying and installing a NFP A Code 96 exhaust hood; fabricate duct work; supplying and installing roof curb and fan; supplying and installing stainless steel back panels; welding and cutting to customize; and otherwise installing fixtures or other personal property or equipment which was intended to become part of the Premises. 8. The amount claimed to be due is Eight Hundred Seventy Six Dollars and No Cents ($876.00), as set forth on the invoice attached hereto as Exhibit "A" and incorporated herein by reference. 9. On October 12, 2000, the Plaintiff/Claimant served the Notice required by Section 50l(b) of the Mechanics' Lien Law of 1963,49 P.S. l50l(b), upon the Defendant(s)/Owner(s) by Certified Mail, Z 429 428 800, and said copy was signed for on October 19, 2000. 10. A true and correct copy of the Notice as well as the Return Receipt card from the United States Postal Office referred to in Paragraph 9 of this Mechanics' Lien Claim is attached hereto as Exhibit "B" and incorporated herein by reference.
