CUSTODY ACTION
ORDER OF COURT
AND NOW, this 12~day of December 2000, the parties having
entered into a Custody Agreement dated November 28, 2000, it is
ORDERED AND DECREED that the custody of their minor child, Briana
Loren Collins, born May 6, 1993, shall be as follows:
1.
Mother shall have legal custody of the child.
Father
shall have equal access to the child's personal, school and medical
records and each party is to inform the other of significant events
that occur in the child's life.
2. Mother shall have primary physical custody of the child
subject to Father's liberal periods of partial custody:
A. Every other weekend starting November 3, 2000, from
Friday at 6:00 p.m. until Sunday evening at 6:00 p.m..

B. With at least 30 days prior notice, four (4)
uninterrupted weeks throughout the year, no more than two (2) weeks
consecutive and must include his weekend. Mother shall also be
entitled to have four (4) uninterrupted weeks, no more than two (2)
consecutive and may not include more than one of Father's regularly
scheduled weekend.
C. Father may have the child at other times as agreed
by the parties.
3. Mother and father shall alternate the holidays of Easter,
Memorial Day, Independence Day and Labor Day. Father shall have
Easter and Independence Day in even years and Memorial Day and
Labor Day in odd years. Mother shall have Easter and Independence
Day in odd years and Memorial Day and Labor Day in even years.
Unless otherwise agreed by the parties, Mother shall have
Thanksgiving each year. Each year Mother shall have the child on
Mother's Day and Father shall have the child for Father's Day.
Each holiday will begin at 6:00 p.m. the day before the holiday and
end at 7:00 p.m. the day of the holiday. The holiday schedule
shall supersede the regular schedule.
4. Christmas shall be shared. Father shall have the child
each year from 12:00 p.m. until 6:00 p.m.. Mother shall have the
child each year from 6:00 p.m. December 24 until 12:00 a.m.
December 25.

5. Mother and Father shall share transportation with the
receiving parent providing the transportation.
6. This Agreement may be modified by mutual agreement of the
parties.
7. If the parties are unable to agree on an acceptable
custody schedule, they agree to seek the assistance of a custody
conciliator. The cost of the custody conciliator shall be equally
shared by the parties.
8. Both parents understand the need to provide a loving and
stable environment for their child.
9. Mother and Father agree to place any personal differences
aside and work for the best interests of the child.
10. The parties shall provide for reasonable telephone access
to the child.
11. Neither party nor persons in the presence of the child
shall make derogatory comments about the other.
12. Each party is to inform the other of any change in
employment, residence and/or telephone number.
13. This Agreement shall supersede any other Agreements or
Stipulations concerning the custody of the child.

CUSTODY AGREEMENT

and between Aleta V. Minium, hereinafter referred to as Mother, and
B~ian L. Collins, hereinafter referred to as Father.
WITNESSETH:
WHEREAS, Mother and Father are the parents of Briana Loren
Collins, born May 6, 1993; and
WHEREAS, Mother and Father wish to enter into this Custody
Agreement to settle and resolve the outstanding issues regarding
the custody of their child;
NOW THEREFORE, it is agreed by the parties that:
1.
Mother shall have legal custody of the child.
Father

records and each party is to inform the other of significant events
that occur in the child's life.
