Andrews, Esquire, request that this Court Order a change of name for her pursuant to 54 Pa. CONS. STAT. Section 701 et. seq. and in support thereof respectfully represents as follows: 1. Petitioner is an adult individual having been born October 12, 1982. 2. Petitioner AMBER LEIGH EBERHART, is a resident of Cumberland County, Peunsylvania and has resided at the following locations during the five years next proceeding the filing of this petition: 33 Victoria Way, Camp Hill, P A 17011 from September 1997 to present; and 503 Erford Rd., Camp Hill, PA 17011 from February 1996 until September 1997; and 722 North Market Street, Selinsgrove, P A 17870 from 1994 to February 1996. 3.
