Travis, respectfully represents: 1. Your petitioner was born Idamae Ruth Reed, by marriage she took the name Idamae Ruth McClintock. 2. Petitioner does not like her first name "Idamae," and has been accustomed to using the name "Cookie" McClintock for as long as she can remember, and has used the name "Cookie McClintock" professionally. Additionally, the name Idamae is difficult to pronounce and spell. 3. That petitioner has resided at 3 Timber Road, Mechanicsburg, Cumberland County, Pennsylvania 17050 during the five years preceding the filing of this Petition. 4. That there are no judgements or decrees dflike character of record against petitioner.
