Le Thi Nguyen is an adult individual, born in 1957 in the former Republic of Viet
Nam to Tu Van Nguyen (father) and Le Thi Loi (mother).
2. In 1982, Le Thi Nguyen emigrated to the United States.
3. In 1988, Le Thi Nguyen was naturalized as a United States citizen.
Le Thi Nguyen is a resident of Cumberland County Pennsylvania, owning a home
at 5245 Joshua Rd. Mechanicsburg, PA 17055.

Le Thi Nguyen wishes, for reasons of phonetic convenience and in order to
facilitate business and personal communications, to change her given name to
"Veronica".
WHEREFORE, Le Thi Nguyen respectfully request that this court issue an order
changing her name from Le Thi Nguyen to Veronica Thi Nguyen.

INSTRUCTIONS
Due to the vastness of our fingerprint records, it is imperative that the complete classification
formula be employed for searching and filing in each instance. Accurate classifying depends primarily
upon the best possible rolled impressions that can be taken.
Fully rolled, clear impressions allow for accurate pattern differentiation, ridge counting, whorl
tracing, and interpretation of whorl types.
It is suggested that each newly completed fingerprint card be examined to ascertain if it can be
fully classified, bearing in mind the following: (1) loop-type patterns cannot be classified unless the
center of the loop, the delta, and the ridges between them are clear; (2) whorl-type patterns cannot be
classified unless the deltas and the ridges connecting them are clear; (3) arch-type patterns can be
classified as such only if a sufficiently clear impression is obtained to permit identification of the
pattern as belonging to the arch category.
While a concerted effort is made to process each fingerprint card forwarded, in some instances
this is not possible. The PSP fully recognizes the occurrences of situations which challenge the
ingenuity of the identification officer to secure legible impressions. All returned fingerprint cards do
not necessarily reflect upon the ability of the operative taking the prints and no returns are made at
any time with such a thought in mind.
Your cooperation is solicited in obtaining the best possible impressions in each block on
every fingerprint card submitted. By so doing you are rendering a real service and making a major
contribution to ali agencies participating in the fingerprint exchange program.

PETITION FOR SPECIAL RELIEF
AND NOW comes Le Thi Nguyen (Petitioner) by and through her attorneys
Christopher S. Lucas & Associates and relates the following facts:
1. Petitioner filed a Petition to Change Name and was fingerprinted pursuant to the
processing of that petition.
2. Two separate methods of fingerprinting were utilized (traditional ink and
computer imagery).
3. Efforts were made to enhance the print using Windex as well as hand cr~me
which the technicians indicated can help to "bring out" the prints.
4. Nevertheless, the Pennsylvania State Police were unable to process Petitioner's
fingerprints.
5. The State Police noted that the inability to read or process the prints may be "due
to the nature of [Petitioner's] employment."

Petitioner is self employed and the owner of Jim's Deli on Pine Street in
Harrisburg. Her job involves cooking and washing dishes from 6:00 a.m. when
the deli opens until 4:00 p.m. when it closes every day.
7. It is impractical for her to stop working long enough to let the skin on her hands
grow out.
WHEREFORE, Petitioner asks that the court process her request to change her
first name without fingerprint processing.
