CLAIM BY SUB-CONTRACTOR FOR VALUE OF LABOR
PERFORMED AND MATERIALS FURNISHED
To The Prothonotary of Cumberland County:
Claimant, Kenneth B. Lelunan, of the Township of North Newton, Newville,
Pennsylvania, carrying on a business at 105 Doubling Gap Road, in the said city of
Newville, files his claim for the sum of Two Thousand Five Hundred Dollars and 00/100
($2,500.00) together with interest and costs against Farmers Trust Company, Guardian of
Terry 1. Bishop, and their agent, Terry Radabaugh, contractors and against all that
certain dwelling at 799 Creek Road, Carlisle, for certain materials furnished and labor
performed in doing the iron work in and about the said dwelling and makes the following
statement of his demand:
1. The name of the party claimant is Kenneth B. Lelunan, and he files this
claim as subcontractor.
2. The name of the owner lJf said property at the time of the furnishing of
said material and labor 'and the attaching of the lien therefore is the
aforesaid Farmers Trust Company, Guardian of Terry 1. Bishop, and their
agent, Danny Radabaugh, and the property is 799 Creek Road, Carlisle,
Pennsylvania, 17013.

3. The date of the completion of the claimant's work was September 22,
2000.
4. The kind and character. of the labor furnished by the claimant and the
prices charged therefor is as follows:
Fabricating iron railing, i!nstalling, priming and painting railing.
5. The kind and character of the materials furnished by the claimant and the
prices charged therefor is as follows: fabricated iron railing, installing,
priming and painting same. Cost of material and labor total $2,500.00.
6, The amount claimed to be due is $2,500.00
7. The materials and labor furnished in and about the aforementioned
dwelling were according to a certain contract between Farmers Trust
Company, Guardian of Teny L. Bishop, and their agent, Danny
Radabaugh, the contraptor, and said Kenneth B. Lehman, the
subcontractor, entered into on or about September 10, 2001.

8. The delivery of the said materials and the furnishing of said labor
commenced on or about (September 15,2000, and the said Two Thousand
Five Hundred Dollars artd 00/100 ($2,500.00) is the balance due on the
contract price as agreed upon by said contractor for the furnishing of the
said materials and labor at the request of said owner and furnished to and
supplied for and towards the aforementioned dwelling and on the credit of
the said building continuously from September 15, 2000 to September 22,
2000, inclusive, in accordance with and in fulfillment of the said contract.
The lien is against the fee and is claimed from September 22, 2000.
9. The said claimant has made repeated requests of said general contractor
for the payment to him of the said balance of Two Thousand Five
Hundred Dollars and 00/100 ($2,500.00) due him on said contract, which
requests said contractor has ignored and refused to comply with.
ro. The total sum to be paid to the said claimant by the said Farmers Trust
Company, Guardian of Teny L. Bishop, and their agent, Danny.
Radabaugh, under the terms of the said contract up to and including
September 22,2000, for the furnishing of the said labor and materials was
Two Thousand Five Hundred Dollars and 00/100 ($2,500.00).

The said claimant therefore clfims to have a lien upon the said dwelling as
hereinbefore described for the amount of the claim, the balance now due, the said sum of
Two Thousand Five Hundred Dollars ~d 00/100 ($2,500.00), from the said 22nd day of
September, 2000, according to the acts qf assembly in such cases and made and provided.

Kenneth B. Lehman, the party claimant named in the foregoing mechanic's lien
claim, being duly sworn according to the law, deposes and says that all the facts therein
set for are true, so far as they are within his knowledge and so far as they are derived
from information obtained from others; that he has made careful inquiry as to the truth
thereof, and as a result of such examination and inquiry he believes them to be true, and
expects to be able to prove the same upon the trial of this case.
