Sahi, through his counsel, Kevin D. Gillespie, Esquire, and BOSWELL, TINTNER, PICCOLA & ALFORD, hereby requests this Honorable Court order a change of name pursuant to 54 Pa.C.S.A. § 701, et seq., and in support thereof represents as follows: 1. Petitioner, Herinder S. Sahi, was born on October 18, 1972, and is over the age of twenty-one (21) years. 2. Petitioner, Herinder S. Sahi, is a resident of Mechanicsburg, Cumberland County, Pennsylvania, and has resided at the following locations during the previous five (5) years: a. Petitioner currently resides at 1740 Eliza Way, Mechanicsburg, Cumberland County, PA 17050 and has done so since June 2006; b. For all times prior thereto, Petitioner's permanent address had been 30 Farmhouse Lane, Camp Hill, Pennsylvania 17011. 3. Petitioner, Herinder S. Sahi, is a graduate of Jefferson Medical College in Philadelphia, Pennsylvania and is currently an internal medicine resident at Harrisburg Hospital, Harrisburg, Dauphin County, Pennsylvania. 4. Over the course of his lifetime, and significantly, during the course of the studies in his profession, Herinder S. Sahi, has been identified as "Harry S. Sahi," by his colleagues and friends. Currently, Petitioner's medical license and medical school certification are in the name of "Harry S. Sahi." (See Exhibits "A" and "B," copies of same, attached hereto). This identification has been the preferred identity of the Petitioner. 5. Because Petitioner wants to continue to be referred to as "Harry S. Sahi," and wishes this identity to be that indicated in all future licenses, certifications and other documentation associated with his profession, as well as all other personal identifying material such as his driver's license and his passport (copies of Petitioner's current license and passport have been included herewith as Exhibits "C"and "D," respectively, both in the name of "Herinder S. Sahi," as well as Petitioner's Birth Certificate, a copy of which is also attache hereto, and marked Exhibit "E"), Petitioner respectfully requests that this Honorable Court enter an order legally changing his name to reflect that which he has become known as over the years. 6. Petitioner has no judgments or decrees of like character of record. 7. Petitioner has had no prior criminal convictions for either felony or misdemeanor offenses, and faces no pending charges of a like nature. Petitioner has included his fingerprints as part of the instant petition so that an appropriate investigation may be completed to verify this averment (see Exhibit "F"). -2- 8. Petitioner has no other illegitimate purpose for seeking the requested change of name. WHEREFORE, Petitioner, Herinder S.
