JUDGMENT NOTE
JOHN M. TODD and SillRLEY A. TODD, husband and wife, known as "Makers", promises to
pay to HCR MANORCARE HEALTHCARE SERVICES, as "Holder", of 940 Walnut Bottom Road,
Carlisle, PA., 17012, its successors and/or assigns, the principal sum of Fourteen Thousand Three
Hundred Twenty-one and 58/100 ($14,321.58) with interest at the rate of six percent (6%) per annum
from the date hereof. Payments shall be made in the amount of $100.00 per month with the first
payment due on the first day of the month following the date hereof and thereafter on the first of each
month for five (5) years at which time, all remaining principal and interest shall be due and payable.
Makers shall have the right to prepay all or any part of the principal amount ofthis Note at any time or
from time to time without premium or penalty, provided that Makers pay at the same time all interest
accrued thereon to the date of payment. In addition, Makers do hereby authorize and empower the
Prothonotary or any Attorney of any Court of Record of Pennsylvania or elsewhere to appear for and
enter judgment against them for the above sum, with or without declaration, with costs of suit, release of
errors, without stay of execution, and with 15 percent added for collection fees; and Makers hereby agree
not to make any motion or any application whatsoever to any Court for an inquisition on any real estate
that may be levied upon to collect the aforesaid sum, and Makers voluntarily condemn same, and
authorize the Prothonotary to enter said voluntary condemnation upon the Writ of Execution. Makers
further agree that any property, real personal or mixed may be sold through a Writ of Execution and
further hereby waive and release all relief from any and all appraisements, stay or exemption of any State
now in force or which are passed hereafter. NO SINGLE EXERCISE OF THE FOREGOING POWER
TO CONFESS JUDGMENT SHALL BE DEEMED TO EXHAUST THE POWER WHETHER OR
NOT ANY SUCH EXERCISE SHALL BE HELD BY ANY COURT TO BE VALID, VOIDABLE, OR
VOID, BUT THE POWER SHALL CONTINUE UNDIMINISHED AND IT MAY BE EXERCISED
FROM TIME TO TIME AS OFTEN AS THE HOLDER HEREOF SHALL ELECT, UNTIL SUCH
TIME AS THE HOLDER HEREOF SHALL HAVE RECEIVED PAYMENT IN FULL OF ALL
AMOUNTS OWING HEREUNDER, TOGETHER WITH COSTS.

KNOWINGLY AND VOLUNTARIL Y WAIVE OUR RIGHT TO CONTEST THE ENTRY OF THIS
JUDGMENT AGAINST US IN COURT AND DO HEREBY CONSENT TO THE ENTRY OF THE
