The minor, Michael Tylor Bollinger, was born on October 22, 1997 and has continuously resided with the Petitioners since early 1999; primary physical custody of the minor was confirmed in petitioners by Order ofthis Court, Oler, J., on Apri129, 1999, in proceedings fIled to Number 99-1539 Civil Term. 4. The minor's father is Christopher Hamilton Spankuch, Petitioners' son, and he has a mailing address of P.O. Box 36, Wellsville, PA 17365. 5. The minor's mother is Peggy Sue Bollinger, who resides at 7812 Jonestown Road, Harrisburg, P A 17112. 6. The minor's father and mother were never married. 7. The minor resided with his mother for a brief period of time following his birth. 8. Since April 1999, the minor's natural parents have had irregular visitation with him. 9. Your Petitioners desire to change the minor's name from Michael Tylor Bollinger to Michael Bollinger Spankuch because of his strong integration into his paternal grandparents' family.
