Johnson, Esquire, request that this Court Order a change of name for him pursuant to 54 Pa. CONS. STAT. Section 701 et. seq. and in support thereof respectfully represents as follows: I. By the time a hearing is held on this matter, the petitioner will be an adult individual having been bom July 12, 1983. 2. Petitioner VINCENT JOSEPH MARCHESE, is a resident of Cumberland County, Pennsylvania and has resided at the following location during the five years next proceeding the filing of this petition: 840 North Pitt Street, Carlisle, PA 17013 (12 years) 3. Petitioner requests that his name be changed from VINCENT JOSEPH MARCHESE to VINCENT JOSEPH CASEY. 4. The reason for the requested change of name is that Petitioner has resided most of his life with his mother and stepfather whose last name is Casey. He has used the name, VINCENT JOSEPH CASEY, since entering the second grade of school and all of his school records reflect that name. ,~~W'>" " - ->~~ .
