JESSE ROBERT DOMENE

PENNSYLVANIA
Petitioner, Lynsie Domene, on behalf of her minor son, Jesse Robert
Domene, requests that this Court order a change of name for her son, and in
support thereof, represents as follows:
1. Petitioner, Lynsie Domene, is an adult individual residing at 66 Old
Stonehouse Road, Carlisle, Cumberland County, Pennsylvania 17013-9785.
2. Petitioner is the biological parent of one child, Jesse Robert Domene,
a minor born out of wedlock on September 16, 1990. Jessie is 11 years of age.
3. Petitioner has resided at 66 Old Stonehouse Road, Carlisle, PA since
September 24, 2001.
4. Petitioner previously lived at 700 Lisburn Road Apartment 1, Camp
Hill, Pennsylvania 17011 from approximately July of 2000 until of August of 2001.
5. The biological father of Jesse Robert Domene is Terry W. Hurley, who
died on August 26, 1993.
6. Jesse, who was almost three (3) years old at the time of his father's
death, has been in the legal custody of Petitioner since his birth.
7. Jesse has resided with his maternal grandparents, Ann and Clyde
Kramer continuously since June, 1995.
8. Petitioner, on Jesse's behalf, requests the addition of Hurley, Jesse's
natural father's surname, changing his current surname from Domene to
Domene-Hurley.
9. Petitioner believes and, therefore avers, that the change of her son's
surname from Domene to Domene-Hurley is entirely consistent with Jesse's
wishes and will be in Jesse's best interests.
10. Jesse, for the past year, has expressed his strong and unwavering
desire to assume his father's surname.
11. Jesse and Petitioner believe that the addition of his father's surname
will provide him with a better sense of connection with his deceased father.
12. Jesse receives Social Security benefits as a result of his father's death.
13. Petitioner firmly believes that Jesse intellectually and rationally
understands the significance and permanence of having his father's surname
added and that Jesse truly wishes to have the change decreed by this Court.

WHEREFORE, Petitioner respectfully requests that this Honorable Court
enter a decree changing the name of her son from Jesse Robert Domene to Jesse
Robert Domene-Hurley.

September 16, 1990, is hereby changed to Jesse Robert Domene-Hurley.
