Faller, hereby files its claim for sewer services against the real estate hereinafter described
and sets forth its claim as follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed is Lester R. Conrad of 112 Meeting House Road, Carlisle,
Pennsylvania.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit" A" with an address of 112 Meeting House Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the
amount of $4,700.00.
6
Interest is claimed on the above-mentioned amount from October 19, 2006.

Cumberland County, pennsylvania, hereinafter GRANTBE
WITNESSETH that in consideration of the sum of FIFTY-ONE THOUSAND
~nd XX/100 ($51,000.00) DOLLARS in hand paid, the receipt whereof
1S hereby acknowledged, the said Grantors do hereby grant and
convey unto the said Grantee, his heirs and assigns,
ALL THOSE CERTAIN two tracts of land situate in North Middleton
Township, Cumberland county, Pennsylvania, more particularly
bounded and described as follows, to wit:
TRACT NO.1
BEGINNING at a point in the western line of the Meeting House
Springs Road, said point being distant 360 feet, measured in a
northerly direction, from the intersection of the northern line
of Louther street extended and the said western line of the
Meeting House Springs Road; thence at a right angle to the line
last mentioned in a westerly direction a distance of 154 feet,
more or less, to a point in the eastern line of a twenty foot
alley; thence in a northerly direction along the said alley a
distance ,of 60 feet, more or less, to a point; thence in an
easterly direction and parallel to the course and distance first
above mentioned a distance at 150 teet, more or less, to a point
in the western line of the Meeting House Springs Road: thence at
right angle to the course and distance last mentioned and along
the western line of the said Meeting House Springs Road a
distance of 60 feet to the point and place of BEGINNING.
BEING the same premises which Doris I. Rhoads by her deed dated
the 24th day of July, 1992 and recorded in the Office of the
Recorder of Deeds in and for CUmberland county in Deed Book U,
Volume 35, Page 554, granted and conveyed to James E. Ciccolella,
one of the Grantors herein. The said James E. cic90lella being a

one of the Grantors herein. The said James E. Cicco1ella being a
single man at the time of purchase, but has since married, his
wife, Kathleen Ciccolella, executes this deed to convey any and
all of her right, title and interest in the property, as she may
have individually or through her marriage.
TRACT NO.2
BEGINNING at a point on the Heeting House springs Road, which
point is then northeastern corner of Tract No. 1 herein; thence
in a westerly direction along aforesaid Tract No. 1 150 feet to CI
20 feet proposed alley; thence along the latter in a northerly
direction 10 feet to lands now or formerly of Haggie B. Ellerman;
thence along the latter in an an easterly direction 149 feet to
the Heeting House Springs Road aforesaid; thence along the
latter in a southerly direction 10 feet to a point, the place of
BEGINNING.
BEING the same premises which Doris I. Rhoads by her deed dated
the 24th day of July, 1992 and recorded in the Office of the
Recorder of Deeds in and for Cumberland County in Deed Book V,
Volume 35, Page 554, granted and conveyed to James E. Ciccolella,
one of the Grantors herein. The said James E. Ciccolella being a
single man at the time of purchase, but has since married, his
wife, Kathleen Ciccolella, executes this deed to convey any and
all of her right, title and interest in the property, as sh€ may
have individually or through her marriage.
AND BEING improved with a residential dwelling known and numbered
as 112 Meeting House Road, Carlisle, Pennsylvania 17013.
And the said Grantors will specially warrant and forever defend
the property hereby conveyed.
